1. What is cryo-therapy?

Cryo-therapy means exposing the body to a medium with a lower temperature so that heat is extracted from the body. It is a local, heat extracting physio technical application in preparation for physiotherapy or an independent therapy.

2. Physiological reactions in case of trauma

The cell damage resulting from an acute trauma causes a release of chemical substances which, in turn, cause an inflammatory process so that the haematoma can be removed.
A normal reaction of the body to inflammation consists in an increase in the metabolism, as a result of which the tissues need more blood. But since the network of blood vessels has been damaged, there is a lower blood circulation in the tissues near the injury. That results in a lower oxygen supply to the cells near the primary injury and thus a secondary damage is caused: the total amount of damaged tissue increases and more tissue rests are added to the haematoma.

2.1. Effect of cold on injuries

Cold immediately applied after a trauma will make the tissue temperature drop, causing a deceleration in the cell metabolism. That results in a decreased need of oxygen so that the cells are more resistant to the period of hypoxia. That decreases the scope of the secondary damage and thetotal amount of oedema will be smaller. Therefore it is important that cold is applied as soon as possible to speed up the recovery process.

3. Effect of cryo-therapy on the temperature of the various tissues

The common factor around which everything revolves is the presence of a thermal gradient. That means that both deep and superficial tissues conduct heat to the place where cold is applied. That way, the heat loss in the place that is cooled off is compensated.

3.1 Skin temperature

When applying cold, the skin temperature will drop considerably on condition that the temperature of the cold analgesic remains constant. When the cold analgesic is removed, the skin temperature will start to rise immediately. The subcutaneous temperature shows more or less the same course as the skin temperature.

3.2 Intramuscularly temperature

There are different views on this issue:

  • The decrease in intramuscularly temperature is believed to occur immediately16
  • It is believed that the intramuscularly temperature hardly decreases or does not decrease at all during the first minutes following the cold application (= delayed drop)
  • It is believed that there is an initial temperature increase. After the removal of the cold analgesic, the temperature keeps decreasing for a while (= after fall).

3.3 Influencing the decrease in temperature The temperature change of the various structures depends on:

  • The duration of the application
  • The cold analgesic used
  • The temperature interval
  • The contact area of the cold analgesic
  • The degree of blood flow
  • The heat storage capacity of the cold analgesic

4. Cryo-therapy and the blood circulation in the tissues

It appears that when cold is applied immediately after an acute injury has been incurred, swelling is prevented by a decrease in the blood flow (= vasoconstriction). When cold is applied during rehabilitation, the recovery is claimed to be furthered by a cold induced increase in the blood flow (= vasodilatation). It seems absurd that one day cold application decreases the blood flow, whereas the next day it appears to do exactly the opposite, only because the reason for cold application changes.

4.1 What should be done in case of acute tissue traumata?

According to Knight, we ought to concentrate on the metabolic effects (metabolism, reduction of secondary damage) and the effects of cold application on pain (pain and muscular spasms, early mobilization and speedy recovery), whereas we ought to ignore the circulatory effects.

5. Cold-induced pain

Clinical observations leave no doubt that cold application is very effective in pain control. But it is an established fact that local cold application can cause severe pain initially.
Some people believe that the pain is caused by an increase in the thermal gradient between the superficial temperature and the temperature of the deeper tissues. When the skin is gradually cooled down to a certain temperature, the temperature gradient can never be high enough to cause pain.
The Cryo-Human "The Next Generation" makes it possible to make the temperature fall slowly so that an adaptation is aimed at and so that the patient does not suffer any cold-induced pain.

5.1 Pain inhibition as a result of cold therapy

There are a couple of theories about pain reduction or pain suppression by means of cold application:

  • Cold causes a decrease in the nerve conduction velocity ? blocking or deceleration of the transmission of pain impulses.
  • Cold functions as a “counterirritant”; the central pain receptors are flooded with cold impulses ? pain impulses are suppressed and obliterated.
  • Cold leads to a decreased activity of the striated muscles inhibiting the stretch reflex ? reduction of painful muscular spasms.
  • Cold suppresses the sensitivity of free nerve endings and of peripheral nerve fibres ? higher pain threshold.

The device

The Frio-Horse cooling system has an internal cooler that cools a certain amount of liquid. The temperature is controlled by a PLC, and can be changed as desired by you. The lowest possible temperature is 2°C, which is obtained by the constant circulation of the temperature controlled coolant that circulates through the cooling pads. This liquid is cooled, monitored and controlled by our ingenious system, so the device is very safe.
